Instructions
1. Notice that each of the Ten Dilemmas has A, B, C, and D choices.

2. If you want to print a copy of your survey for yourself, go to the BOTTOM of the survey and click "Printer Friendly." Then take the survey on the Printer Friendly form.

3. Complete the survey by checking none, any, or all four choices (A, B, C, D) under each of the Ten Dilemmas.

4. BEFORE clicking the "Submit" button below the survey, click "File" in the upper left-hand corner of your screen, then click "Print".

5. Your computer may ask you to choose a printer. Do so. Then click "Print."

6. When printing is complete, click "Submit" to send your survey.  You will return to the Home Page at StarkTelecoaching.com.
